with a required 50% in Division I

Minimum of 1 KSA activity (Knowledge Self-Assessment)
Minimum of 1 Performance Improvement activity (based on patient population)
Minimum of 50 Family Medicine points from completion of Self-Assessment and Performance Improvement Activities

One (1) Family Medicine Certification Process Payment
Submission of application and accompanying full examination fee for the Family Medicine Certification Examination
Successful completion of the Family Medicine Certification Examination
